The Alberta Lacrosse Association carries Insurance for all registered players (minor or major) in lacrosse for the year they are registered.
If a claim needs to be made please report the incident as soon as possible to your Coach and to the Executive Director (executivedirector@hornetslacrosse.com).
The ORIGINAL claim forms are to be mailed to the ALA office, #4 - 9 Chippewa Road, Sherwood Park, Alberta, T8A 6J7, Attention: Lisa Grant. PLEASE DO NOT SEND CLAIMS DIRECTLY TO THE INSURANCE COMPANY. The ALA will then forward the claims to the Insurance Company and the Insurance Company will then communicate with you regarding your claim. ALL CLAIMS ARE TO BE SUBMITTED 30 DAYS FROM THE INCIDENT.
